Vietnam is one of the few countries that uses a 2:3 portrait aspect ratio, not a square or 35 x 45 mm. Photos prepared for the US, China, or Schengen will be rejected at upload.

The portal at evisa.xuatnhapcanh.gov.vn doesn't tell you what failed
If a file is rejected at upload, you see a red error without specifics. Test the upload step before paying — the $25/$50 fee is non-refundable, and if the photo is rejected after submission, you have to reapply from scratch.
Don't use a CVS/Walgreens passport photo
US pharmacies prepare 2 x 2 inch square photos. Resizing one to 4 x 6 cm leaves the head positioned wrong and the background cropped incorrectly.
For Visa on Arrival or in-person embassy visas, the format is different (2 x 2 inch printed copies).
